Once you submit this form, you will receive an email with a great deal of information, along with links that will be needed to register for Baptism Preparation and to pick a Baptism date. If you don't receive the email (a no reply email from eCatholic), please check your junk/spam folder. Please read through the entire email.

If you do not live within Chatham, you will need to provide a Letter of Permission from the Pastor at your local parish, as well as complete Baptism Preparation with that parish and provide us with a certificate or letter of completion. These documents must be handed into the office before the baptism can proceed.
